Community Health Worker, CSC

Purpose

The Community Health Worker Program at Mountain Empire Community College prepares you to be a trusted advocate, bridge-builder, connector, educator and motivator who helps residents access the health and social services they need to live better, healthier lives.

Program Learning Outcomes

Upon successful completion, students will be able to:

  1. Operate as an advocate for the clients, family, community, and populations across the lifespan and on behalf of diverse populations to promote health and prevent disease.
  2. Incorporate professional communication with the population served while collaborating within an interdisciplinary health care team.
  3. Demonstrate competencies related to basic health education knowledge across the lifespan for the person experiencing physical, emotional, mental, and/or spiritual disruptions.
  4. Analyzes holistic health and wellness needs while applying knowledge to connect individuals, families and groups with the appropriate resources.
  5. Demonstrates professionalism within the scope of practice of a community health worker (CHW)

Employment Opportunities

Community Health Workers can take on a variety of job titles such as:

  • Community Care Coordinator
  • Community Health Advisor/Representative
  • Community Resource Navigation Specialist
  • Health Coach
  • Neighborhood Health Advisor
  • Outreach Worker/Specialist
  • Patient Navigator
  • Peer Counselor/Educator

Program Requirements

An academic advisor will review with students their preparation in Mathematics and English. Students will be guided to appropriate preliminary coursework to give them the greatest chance of success in this program.

Additional Information

Community Health Worker is not a competitive admissions program, but an application is required. The deadlines to submit a program application for the fall cohort is July 15th and October 15th for the Spring cohort.

  • State accredited high school diploma, home school diploma, or GED.
  • Students must have English and Mathematics skills assessed. Deficiencies in these areas may be corrected by completing the appropriate developmental study courses. All identified developmental course needs must be completed prior to admission in the Community Health Worker program.
  • Demonstrated competency in Math and English by one of more of the following:
    • Placement out of or completion of MDE 10.
    • Students must demonstrate competency in English 111 with placement into or completion of ENG 111.
  • Immunizations as required
  • Some internship sites may require a criminal background check
  • Drug test
  • Current CPR certification
  • GPA requirement of 2.0 or higher
